| 1. | Have Prior Approval Form completed and signed by your school superintendent or designee. |
| 2. | Turn Prior Approval Form in at the Staff Development Desk in the conference registration area at the Macon Centreplex (if you haven’t mailed it already). Pick up a copy of the Workshop Attendance Confirmation Form and/or Concurrent Sessions Confirmation Form to be completed during the conference. |
| 3. | Attend workshops and concurrent sessions at the conference and have your Workshop Attendance Confirmation Form and/or Concurrent Sessions Confirmation Form stamped by the facilitator in EACH workshop or concurrent session that you attend. |
| 4. | When leaving the conference, turn in your completed Confirmation Form(s) along with a completed Conference Evaluation Form to the Staff Development Desk at the Centreplex. Pick up a copy of the Staff Development Unit (SDU) Course Completion Form to be completed after the conference. |
| 5. | After the conference, participants
return to their workplace and begin implementation strategies learned at
the conference. By or before May 1, 2004, the SDU Course
Completion Form must be completed, signed and returned to:

| 6. | Your SDUs will be awarded and reflected in a transcript that will be mailed to your district staff development office in July 2004. The number of SDUs will be based on your participation at the conference but will be no more than two SDUs. |
